Dues to be Waived: Process and Documentation Required

MEMBERS OF HAMILTON LOCAL 548

UPDATED : APRIL 26, 2013

Dear Sisters and Brothers -

When applying to have your dues waived please keep in mind the following- Dues are waived only in full months when you receive 70% or less of your regular wage. Members who use “top-up” credits must wait until the “top-up” credits have expired before their dues can be waived. This criteria was established by the membership of Hamilton Local 548. Members who receive WSIB payments of 75% of their regular wage do not qualify to get their dues waived under this criteria.

The form to apply to waive your dues is available on this website under “Resources”, then by clicking on “Submit forms”. Please fill the form out as completely as possible, and send it to the attention of the Secretary/Treasurer prior to the start of your leave. As this is not always possible, please attempt to send in the form as soon as you can.

In addition to filling out the form to apply for a dues waiver, you will be asked to provide supporting documentation. Supporting documentation can be in the form of SAP records (obtainable from your immediate supervisor- the supervisor is required to provide you with this information) showing the start of your leave/STDP; leave forms signed off by the supervisor; proof of Sunlife benefits starting/stopping; and in the case of RSMCs- RSMC vouchers. This is not a complete list, as individual circumstances may vary. As soon as I receive the application to waive dues, I will contact you to discuss which documentation is required.

The Local tries to expedite the processing of dues waivers. We recognize that members experience financial hardship due to illness, injury or other circumstances. Submitting forms and documentation in a timely manner ensures that your application to waive dues can be processed as quickly as possible.

Thank You:

In Solidarity
Sophie Kruk Secretary/Treasurer,
Local 548
